How to Reach the Underpass Fault
The Underpass Fault is in the Underpass, the Zone found far beneath Manhattan under the sinkhole in the Evacuation Zone. You get near the place during Recursions and Iterations, but you may not have been able to reach it right away.
The Fault is located on the Island of Orange Pipes in the Underpass. If you try to reach it before unlocking a particular traversal ability, called Tap to Reveal, Dylan will find himself flummoxed, unable to continue.

To reach the island, you must first complete Tap to Reveal Finish the mission and return to the Underpass when you're ready.
When it's time, you can use a Fast Travel Door to swiftly return to the Underpass. Interact with a door and travel to The Spire Fast Travel Door, if you have it unlocked.

To continue, you can follow a path down to the platform blocks.

Or you can drop off the western edge of the platform with the Fast Travel Door and use Reach to grab onto a ledge. It's more daring this way, and more fun.

Travel over the blocks as you make your way to Orange Pipe Island. Continue until you reach the once-impassable chasm. Leap and dash over the air until you get close enough to Reach your way to the other side.

Here, you'll find an Orientation Point next to an office.

Resource Crate Location
Enter the office next to the Orientation Point on the outskirts of Orange Pipe Island to find a Resource Crate.
Investigate the Vision of Jesse
Continue making your way over to the island.

Resource Crate Location
Look for a Resource Crate on top of a spire on your way to Orange Pipe Island.
Fast Travel Door Location
This Fast Travel Door can be found along the trail of block islands leading to Orange Pipe Island.
Interact with it to unlock the Fault Fast Travel Door.
After reaching the final ledge with the Fast Travel Door, continue following the path west until you touch down on the island.

Progress around the side of the island, using the pipes, until you spot an Orientation Point on a floor below. Drop down to the point, saving your progress.

Keep descending, taking note of the Mold infestation. You'll likely have to clear the Mold Heavies below.

Resource Crate Location
Near the bottom of the shaft in Orange Pipe Island, you'll find a Resource Crate next to some Mold in the corner.
Head through the doorway to the west and progress until you reach a square opening shining with white light.

Hop into the hallway and pass by an Orientation Point, where another vision of Jesse appears.

Head down the hallway until you reach a ledge. Here, the Board speaks, still trying to convince Dylan to stop pursuing Jesse and focus on fixing the Fault alone. That's probably not going to happen.

Enter the Fault
Leap and float over the space to reach the pipes. Drop onto them and cross the next gap to the ol' CRT TV waiting for you on the other side.

Interact with the TV to enter the Dimensional Fault.
Find Jesse and Talk to Her
After entering the Fault, head forward to spot a strange orange silhouette in the distance. It disappears as you approach.

Continue until you reach a ledge where you can spot a shadow on the wall. Dylan mentions needing to find... someone. Jesse had warned him before that being in the Dimensional Fault can cause you to lose memory, and it seems like he's forgetting her.

Progress until you reach a doorway with another silhouette, which also disappears when you get close.

On the next platform, Dylan finds himself surrounded by objects: a bed, a bike, and a bench. He's continuing to forget things, this time losing track of his own name.

A light shines onto a nearby wall, showing the shadows of a person. Here, you can interact with it, causing Dylan to remember his mother.

Another wall lights up, this time reminding him of his dad.

Look at and interact with the final wall. Dylan recalls being happy with his family, mentioning that there were only three of them. But he vaguely feels that isn't correct.

Afterward, a large opening appears in the wall, and that silhouette from earlier appears standing on a ledge within. Leap over the gap to continue.

The silhouette grows larger and takes on more detail on the wall. It runs as Dylan approaches, and he gives chase.
Continue following the silhouette, traveling across Gravity Anomaly surfaces, until you reach another wall with shadow figures.

Interacting with the wall causes Dylan to reminisce about his childhood. He mentions the happiest moment of his life was when he got Severed Circle II, the game he and Jesse adored as children. It's what's playing on every TV in the Dimensional Faults, so clearly it's a precious shared memory with his sister.

After the nostalgia ends, another gap appears with the orange silhouette waiting. Head through and continue. Dylan soon mentions something bad happening, but he can't remember it.

In the area beyond, Dylan finds two silhouettes. He asks them to follow him.

Guide the Light-Projected Children
In this next section, Dylan must help the light-projected children on the walls by leading them to specific destinations.
There are some limitations to their movement. The children must follow Dylan using the walls he's next to, which means they can't follow him over large gaps. You need to help guide them to where they need to go.
In the next section, you can see two ways to reach the area you need to get to: left along the walls, and right over a gap.

If you leap over the gap on the right, the light-projected children will stay on the previous side, stuck.

Guide them along the left path, taking them to the larger shadows.

The shadows light up and animate, and Dylan once again reminisces. Interacting with the new image, he clearly recalls himself and Jesse after an incident.

He remembers that his parents treated them differently afterward: his father drinking, the abuse; their mother hiding away, never leaving her room. After he mentions Jesse running away, another doorway opens.

In the next area, Dylan discovers a lone light-projected child. He remembers being scared and not wanting his sister to be alone, and recalls feeling some kind of force that was trying to bring them back together.

Reunite Young Dylan and Jesse
Your next task is to bring the light-projected Dylan and Jesse back together. After entering the next section, take the light-projected Dylan down the path until you reach a gap you need to clear.

At the end of a path is a wide gap. You have to bring Dylan to the other side.

To begin, look along the path you traveled for a drop down to the road that leads to the cliff. You'll find it, not long after meeting Dylan, on the right side past a gap.

Continue down the path until you reach the bottom of the cliff.

Turn around to spot the ledge from earlier and use Reach to return to it.

By doing this, Dylan stays at the spot you left him. Next, fly over the gap to the top of the cliff edge. This forces Dylan to climb up to reach you.

Now that you're reunited, continue down the trail until you end up at a wall with two shadowy figures representing Jesse and Dylan. The light-projected child fills the right space.

Face the shadows and look to the left for another path.

Follow it until you end up at another area with a large gap. You can see the second light-projected child across the way.

Head down the path on the left side until it goes straight up. This is a Gravity Anomaly surface, so press the associated key or button to latch onto it.

Continue down the path until it leads you to the light-projected Jesse.

Now, you must figure out how to bring Jesse to the path you left earlier, but there's a massive gap in the way!

To solve this issue, bring Jesse to the edge of the spot where you dropped from and then leap over the gap, leaving her at the wall.

Take the path to Jesse a second time. As you travel, you should see her waiting where you left her.

After you reach the wall, she'll leap up and join you on the road.

As you make your way back, you'll find the path splits. One road leads to an edge that you can leap across, but that'll leave Jesse behind. Instead, climb up the short ledges on the right-hand side.

Follow the path back to the wall to reunite the light-projected siblings. Interact with the wall and Dylan remembers the promise he and Jesse made.

After this, a hole appears in the floor. Drop through it and land on the surface below.

In the distance, on top of a platform, is Jesse. Leap over the edge and climb up to reach her.

Speak with Jesse. She's dealing with her own demons, but Dylan can reply to what she's saying. Feel free to pick any dialogue options, as they won't change the outcome of the story.

Help Jesse Complete the Ritual
Jesse begins to recollect a particularly painful memory, even at the protest of Dylan. She turns to view the wall, and the shadowy figures of Dylan and Jesse appear. Interact with the wall to remember.

The memory is a dark one, involving their parents and a gun. At the end, a hole appears in the wall, and Jesse dashes toward it.

Follow Jesse on the other side and continue until you reach another TV.

Interact with it, and the siblings finish the ritual. They also get their memories back. Jesse shares her deep regret over Dylan losing so much of his life at the FBC, and worries that he could get killed doing what he's doing now. You have two options for a reply. Feel free to pick either one.

Continue the conversation, and soon Jesse mentions there's one Dimensional Fault left to close, and then it'll be over.
Finishing the conversation ends The Underpass Fault. You're rewarded with x1 Living Shard, 600 Zone XP, and the collectible: Before I Leave.
